A Case of Antimonium Crud

ADHD symptoms in a 6-Year-Old Boy may suggest numerous remedies, but astute analysis ultimately led to the simillimum.

This child was seen in November 2024 and his main complaint was difficulty concentration with hyperactivity (diagnosed with ADHD) and marked emotional instability. He was better for motion.

He behaves like a younger child, especially when angry. He cannot tolerate heat in summer. After gastroenteritis in July 2024, his hair began to thin. Generally, very aggravated by hunger, especially emotionally (anger).

At 10 months of age, he suddenly stopped digesting mother's milk (and most other foods). The stool became green, he started losing weight, and at that time, he had eczema (backs of the hands and popliteal fossae). He could only tolerate meat and broths.

Later, he was on the GAPS diet for about 2 years, but even on GAPS, he could not tolerate dairy products. The intolerance to dairy products completely normalized after Calcarea Carbonica some years ago. He has a very poor appetite and is picky about food.

Analysis

If you study this case carefully, you will soon realize that many symptoms point to many different remedies. For example, hyperactivity together with aggravation by fasting, intolerance and aggravation by heat could point to Iodum. However, this remedy did not cover the marked mood changes and was contraindicated by the child's poor appetite.

Inability to fix attention together with a history of intolerance to mother's milk could indicate Aethusa, but this remedy does not cover the intense mood changes nor the hyperactivity. Natrum carbonicum could be considered based on intolerance of heat and milk, but again it does not cover the mood changes and hyperactivity.

The most reliable way to start in such complex cases is first to determine which symptoms are the most important – that is, which symptoms most strongly individualize this child with ADHD and distinguish him from other children with the same diagnosis.

What is not linked (non-pathognomonic) to ADHD, yet remains very prominent in this child, is picky eating, intolerance of hot weather, childish behavior when angry and a history of refusal of mother's milk. The only remedy that connects all these symptoms is Antimonium Crudum, for which inconsistency of mood is a keynote and the childish behavior makes this remedy similar to Baryta carbonica which is complementary to Ant-c in many cases).

That is why my first prescription was Antimonium Crudum 200C, single dose.

First Follow-up January 2025

The most significant change is that he has started eating better, has stopped being picky about food, and has become physically stronger (gained some weight).

Mood swings were worse throughout November and December when he had reduced physical activity; now they are better because he has more training sessions, but compared to the past, when he had a similar training load, there is no real change in hyperactivity.

Regarding the hair, there has been no change — still thinning.

An old symptom returned: involuntary urination when the urge is postponed.

He still has a very poor tolerance of hunger.

Analysis

This follow-up was a very interesting moment for the students who witnessed the case. When the mother said that the hyperactivity and mood changes were the same and that the only change was an improvement in appetite and physical strength (with weight gain), many students concluded that Ant-c was a wrong remedy, because it had not affected the ADHD symptoms.

I explained to them that it was, in fact, a correct remedy. Not only had the child improved physically, but the symptomatology had changed from a complex picture pointing to several remedies into a much simpler and clearer one.

How do you mean that? they asked.

Because the child was no longer picky, his appetite was very good and he looked physically better. What remained unchanged was the hyperactivity (need for motion and amelioration by motion), thin hair, aggravation by hunger and amelioration by eating. In addition, an old symptom that had been suppressed a long time ago by Causticum reappeared – involuntary urination when the urging is postponed.

When we take this new totality, it clearly points to one remedy. This situation is the very opposite of the original state. The fact that the symptomatology has changed from confused to very clear is a sign of better health, which can only happen after the correct remedy.

When we look at the present (remaining) totality, it is very clear since all these symptoms are keynotes of Sepia.

Simply put, the less vitality a patient has, the more confused the symptoms will be; and the more vital the organism becomes, the more order, logic and structure we will see in the patient's symptoms.

What we see here is that after the first prescription the symptomatology became simpler and clearer: some symptoms disappeared, some remained unchanged, and one old suppressed symptom reappeared. This new totality – the remaining symptoms together with the reappearing old symptom – clearly pointed to Sepia.

Second Follow-up March 2025

A few hours after taking Sepia, he began provoking conflicts and bringing up things that had happened in the past. He became even more angry and weepy.

Within 24 hours after the remedy, he became markedly oversensitive, and his moods were changing even more rapidly than before the remedy; this state persisted throughout the following day as well.

On the third day after the remedy, his energy decreased significantly, and he even refused to go to training.

After this initial aggravation, his state began to improve. Now his mood swings are much better (mother reports 95% improvement in this area). The mother reports the complete disappearance of hyperactivity and a 50% improvement in concentration.

Involuntary urination disappeared; it occurred only once after an acute cold and has not appeared since.

Marked thickening of hair – mother describes this to be "unbelievable."

After several years, abdominal pain has returned (last present in kindergarten at the beginning of 2023).

Evaluation

The reaction to Sepia shows all the parameters of a deep constitutional response. A short and strong initial aggravation was followed by a significant general improvement in all areas, together with the reappearance of old symptoms. At this point, it was evident that Sepia was the correct remedy, and that it was now necessary to wait.

Third Follow-up May 2025

Two weeks after the previous follow-up, involuntary urination returned, mood swings worsened, and hyperactivity also worsened.

One week later
Symptoms began to subside, and involuntary urination gradually disappeared.
For the past two weeks
He has been completely free of involuntary urination.

Concentration is better; he can engage in activities that he previously could not sustain.

Conclusion

According to his parents and teachers, hyperactivity has completely disappeared, he is like a healthy child. This case was taken and followed as part of Petr's ongoing Practical Clinical Training, which includes live cases and regular follow-ups.
